Setting up Registration and Medical Forms
The Band Camp module allows you to create your ‘Camp Registration’ and ‘Medical’ forms and to manage the registration process.
An example of a camp registration form setup is below. In the Free Text area, you can enter any information about the camp or any details you need that will be displayed at the top of the permission form.
Below the Attending Camp Question are 5 optional questions that you can add to the registration. They are all Yes/No questions. If you would like parents to be able to make a comment on any of the questions, you can place a message in the ‘Instructions’ block which will then show the instruction and give them place to put an optional comment.
The registration form also includes a section to enter the contacts which is automatically added
Using [Student Name] and [Class] in a question will display the student’s name or class in the question.
Setting up Cabins to Allocate to Children
At the bottom of the Band Camp Registration you can add all the Cabins that will be used on the camp. Once students register you will be able to allocate to them to cabins.
Each Cabin can be allocated the number of beds in the cabin. The system will keep prevent over allocation of students to a cabin.

Medical Form Setup
The first 3 text input boxes are used to add any free text to your medical form as per the sample. If no text is entered in an input box that free text area will not be displayed on the form.
The medical form has a pre-set a set of allergies and medical conditions conditions that will be displayed on the form. You may request additional items to be added to the lists if required.
Allergies List
Bee Stings, Dust, Eggs, Kiwi Fruit, Milk (Lactose Intolerance), Medications, Mosquitoes, Nuts, Penicillin, Seafood, Ticks, Other
Medical List
Anaphylaxis reaction, Asthma, Bed Wetting, Diabetes, Eczema, Epilepsy, Fainting, Hay fever, Sleep Walking, Other
Eight additional custom questions can be added to the medical form. If you would like parents to be able to add a comment to a question then insert an instruction into that questions setup instruction box.
